What we will need from you:

 

 • Detailed contact information

 

 • A brief description of the services your     agency provides

 

 • A description of the project

 

 • Confirmation that your board has     approved this project

 

 • Insurance coverage details

 

 • Can your agency provide any     funding,trucking or volunteers?

 

 • Suggested start date

 

 • Suggested finish date

 

 • The type of furnishings you need

Important, please note:

 

• You must submit proof of your agency's
    not-for-profit status in order to apply.

 

• You must make your own arrangements
    for picking up donated item.

 

• PBD is not able to do electrical,     plumbing, or structural work in a facility.

 

• Any portion of the work requiring special     permits and/or licenses will be the     responsibility of your agency.
